Sleep apnea signs a dentist can spot before you do

Most people think of sleep apnea as something a sleep clinic finds after a night wired to sensors. In practice, the first person to notice is often a dentist, for a simple reason: we look inside your mouth for twenty minutes twice a year, and sleep-disordered breathing leaves marks there.

What we see in the chair

None of these is proof on its own. Together they form a pattern that we take seriously enough to ask about your sleep.

  • Worn, flattened or chipped teeth from grinding. Many people who clench and grind at night do so during brief arousals when breathing is disturbed.
  • A scalloped tongue – ridges along its edges where it presses against the teeth. It often points to a tongue that is too large for the space it has, or one that drops back during sleep.
  • A narrow upper jaw, a high palate or crowded teeth. These shape the airway as much as they shape the smile.
  • A large tongue or tonsils, or a soft palate that sits low and hides the back of the throat when you say "ah."
  • Dry mouth, morning headaches, and enamel erosion from acid reflux, which is more common when breathing is obstructed at night.
  • In children: mouth breathing, snoring, dark circles under the eyes, bed-wetting past the usual age, and restless sleep. We assess children's airways from age two.

Why the mouth tells the story

Obstructive sleep apnea happens when the airway narrows or collapses during sleep. The tongue, the jaw and the palate are the walls of that airway. When the jaw is small or set back, or the tongue has nowhere to go, the airway is easier to block. Dentists are trained to read those structures, which is why dental sleep medicine has become a recognized part of screening.

What happens next at Westside

A dentist can screen for sleep apnea but cannot diagnose it – that is a medical diagnosis. At Westside the two sides of the clinic share one chart, so the next step is short.

  • We talk through your symptoms and use a validated questionnaire.
  • If the picture warrants it, Dr. Sabah Jarullah, our family physician, orders a home sleep test: two nights in your own bed with a small recorder. No referral is needed, and the physician assessment is covered under the Alberta Health Care Insurance Plan.
  • The results are reviewed by Dr. Jarullah, and the treatment conversation happens with both of us in the room: CPAP for some, a custom oral appliance for others, and for some people Solea Sleep – a gentle laser treatment of the soft palate that can reduce snoring.

When to ask

If your partner says you snore or stop breathing, if you wake unrefreshed no matter how long you sleep, or if you notice any of the signs above in yourself or your child, mention it at your next visit or book a sleep assessment directly. Untreated sleep apnea is linked with high blood pressure, heart disease and daytime accidents; the treatment is far easier than the consequences.
